This Hayden switch plate (Part #9065-02) is the handle cover plate that frames and protects the on/off switch on a Hayden SuperHose II central vacuum hose. It holds the switch in place at the grip and gives a finished, secure mounting surface for the wiring beneath.
Note that the current replacement for this part is Hayden #38348-2, which supersedes the original 9065-02. If your switch plate is cracked, warped, or missing after a switch repair, fitting a fresh plate keeps the handle switch firmly seated and protected.
